Emergency

Please call during office hours (8:00am – 5:00pm) and our staff will assist you with any questions regarding your care or treatment. When calling during office hours, if you are experiencing a MEDICAL EMERGENCY, ENTER ‘0’ AND OUR OPERATOR WILL ASSIST YOU IMMEDIATELY.

When calling after office hours (5:00pm – 8:00am and on weekends), we ask our patients to call our main office number and listen to the options of how their call can be handled. IIMC (317) 885-2860, CRSM (317) 885-2334, IPCA (317) 883-4736 and IIDC (317) 885-2860.

The patient always has the option of calling 911 or going to the nearest hospital emergency room should they feel they have a medical condition that warrants immediate medical attention.

Refills

Our physicians feel it is very important that you take your medication(s).

In order to refill your medication, it is necessary that you see the doctor yearly for your check-up. We do not refill medications prescribed by other physicians.

Medications are refilled from 8:00 am to 4:00 pm each business day.

Please have your pharmacy phone number, prescription number and the name of the medication ready when requesting a refill.

Pain Medications are not refilled after 4:00 PM or on weekends.

Medical Record Request

Patients may request a copy of their medical records by calling (317) 885-2860.

A Release of Information Authorization form must first be completed by the patient or legal guardian prior to requesting medical records.

PLEASE NOTE: If a patient has granted Power of Attorney to someone, we must see the Power of Attorney before processing the request for medical records.

If the request is for personal reasons, there is a charge. The charge is $15.00 for the first 10 pages. There is an additional charge of $0.25 per page after the first 10.

HIPAA Privacy Officer

A HIPAA Privacy Officer is responsible for developing a HIPAA-complaint privacy program and ensuring privacy policies to protect the integrity of the Personal Health Information (PHI) are enforced.  The HIPAA Privacy Officer will also monitor compliance, report breaches as necessary, and ensure patients’ right’s in accordance with state and federal laws.
